Horizon Close provides a simulated supported living environment to help prepare our service users to return to independent living. We encourage Horizon residents to participate in a range of leisure, social and vocational activities to prepare them for discharge. This service is designed to provide a stepping stone towards independence and prior to discharge, helps to identify community support needs working with the service user and their family to support all parties in their transition home.
During the recent CQC inspection, the Horizon team were highlighted as offering “good practice” with the following observation: “We saw patients being supported to make decisions about the future and helped to develop skills necessary for independent living. This included detailed transition planning and staff accompanying patients to placements in the community as part of the process.”
Horizon Close offers ten individual bungalows, each has an ensuite bedroom, kitchen and living space with access via own front door. The Horizon Centre adjacent to the bungalows provides a communal activities venue for the Horizon and Glenside service users.
Service users and their families are also able to access all communal areas, including the on-site Coffee shop and mature gardens.
